Please fill in the questionnaire form at https://dogmatters.com/bcq and please also sign the training agreement here

If there are any questions that aren’t relevant, just leave them out.

This form should be received at least two (2) days before the appointment. Your appointment may need to be rescheduled if the information is not received within this time.

Refund policy:

If cancelling appointments a minimum of 24 hours’ notice is required to receive a full refund.

Same day cancellations incur a charge of 50% of the total fee.
